Anglican Church –Diocese of Lake Rukwa (ACT-DLR) has 3 years’ experience of implementing two community projects which are Pamoja Tuwalee and Early Childhood Development Project
OVC program from 2013 to September 2016 in Mpanda Municipal Council and Mpanda District Council in partnership with PACT Tanzania. ACT-DLR is working in partnership with PACT Tanzania to implement KIZAZI KIPYA PROJECT: USAID NEW GENERATION PROJECT in Mpanda Municipal Council and Nsimbo District Council.
Kizazi Kipya: USAID New Generation project is the successor to the Pamoja Tuwalee Project, though with a slightly different vision, focusing on HIV and targeting orphans and vulnerable children, as subsets of most vulnerable children (MVC). KIZAZI KIPYA-USAID New Generation program is a new five-year program. For efficient execution of its duties, ACT-DLR is looking for enthusiastic, creative, energetic individuals to fill the positions currently available, based in Mpanda Municipality.

Case Worker Coordinators

Job Title: Case Worker Coordinators
Reporting To:   Case Management Officer.

Job Description:
Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.

Roles & Responsibilities:

  • Supervise Para Social Wokers/Community Case Workers (PSW/CCWs) and provide continuous mentoring and support to ensure delivery of high quality case management services.
  • Disseminate and ensure adherence to case management, child protection, referral and links, parenting, standard operating procedures (SOPs) and other job-aids that facilitate holistic case management at the household/community levels.
  • Ensure case management services delivered address the holistic needs of OVC and caregivers including health, nutrition, education, protection, livelihoods, and psycho-social well-being.
  • Conduct quality step-down training to PSW/CCWs and facilitate monthly meetings in the field to introduce new topics, improve quality, and reinforce guidelines and procedures.
  • Ensure PSW/CCWs’ complete required case management forms and case filing system are well maintained.
  • Work in partnership with local government authorities as appropriate, including but not limited to District Executive Directors, Council Health Management Teams, District Social Welfare Officers, and ward level officials.
  • Create community links for broader community engagement of youth and caregivers in case management, child protection, parenting, etc.
  • Assist in mapping existing community cadres (Community Health Workers, Home-Based Care Workers, Para-Social Workers, etc.) in communities and assist government officials to recruit and select CCWs for the National Integrated Case Management Training for CCWs.
  • Ensure that all PSW/CCWs are oriented in the Child Protection Policy and Code of Conduct; take all cases of child abuse seriously and follow national protocols to ensure timely reporting.
  • Ensure PSW/CCWs are coordinating with local structures including health facilities and Child
  • Protection Committees (formerly known as Most Vulnerable Children’s Committees).
  • Assist in the development of a service directory for social services (in coordination with the Health and HIV services Officer) in implementation areas; update the directory at least once a year.
  • Submit timely updates to the Case Management Officer for inclusion in the quarterly, semi-annual, and annual reports.
  • Document lessons learned and best practices for experience sharing and replication. Perform any other relevant duties as assigned by the Case Management Officer.


Minimum Qualifications

  • Diploma in social science field
  • Minimum of 3 years of relevant experience in at least one of the following technical areas: Social Work, OVC care, health, nutrition, education, protection, livelihoods, and psychosocial well-being
  • Fluent in both written and spoken English & Kiswahili
  • Experience with PEPFAR 3.0 funded projects in Tanzania

Preference for;
Bachelor’s Degree in Sociology, Public Health, or Economic Development, Community Development or equivalent experience. Over 3 year’s relevant experience
